Job summary One Ellesmere Port Primary Care Network is a collaboration of the 6 GP Practices within the town who collectively care for over 72,000 patients. The PCN is looking for an innovative PCN Care Coordinator to work within York Road Group Practice and to work across our wider PCN family. The post holder will work closely with their GP Practice management team, the PCN Leadership Team and the PCN Operational Group to ensure efficient operation of PCN services, with a specific focus on supporting the delivery of the PCNs Vaccination Programme (Flu, Covid-19 and RSV). The post holder will have excellent communication skills and be flexible in their approach, using their initiative to manage conflicting priorities. The right candidate will be confident in using MS Office (Word, Excel, PowerPoint, Outlook) and in learning to use new computer systems. Experience in general practice is desirable but not essential, but the post holder will have good working knowledge of primary care and a can do approach to their work. Gather information and undertake enquires for the PCN and the wider group as and when necessary.
