We have an exciting opportunity to be part of developing the delivery of the Kirklees and Calderdale Dynamic Support Register (DSR). Your role will entail managing referrals, and monitoring people on the Dynamic support register (DSR) and identifying people whose mental health concerns and/or challenging behaviour place them at risk of being admitted to hospital if they do not get the right care and treatment in the community. This is a brand-new role that has been created to ensure that locally we have a robust process that meets the national NHS requirements ensuring that those supported by Learning Disability services have access to the DSR This role will work closely with several agencies to ensure that information on the register is live and communicated effectively to all stakeholders. Main duties of the job You will be a highly motivated and conscientious individual who has the desire to work creatively with people with learning disabilities within the community, providing the highest standards of safe and compassionate care to clients to maximise their potential. You will use your excellent communication, co-ordination and persuasion skills to achieve the best outcomes for the people we support. You will coordinate Dynamic Support Registers (DSR) and chair meetings across both Kirklees and Calderdale, co-ordinating a multiagency response. You will also provide vital education and training in relation to the DSR across multiple agencies to raise awareness and best practice in line with the NHS DSR and CTR Policy (January 2023). You will also provide information to meet national reporting requirements and in support of the Transforming Care Partnership Board and to ensure all requirements set out by the NHS DSR and CTR Policy (2023) are met and evidenced.
