Job Description:
Our client in Telford is looking for an experienced Adult Social Worker to join their Hospital Social Work Team.
What will your responsibilities be?
In this role, you will conduct Enablement Reviews and Care Act Assessments to support individuals in regaining their independence and complete assessments as required by the Care Act. You will assess individuals’ capacity to make decisions and conduct Best Interests assessments when necessary under the Mental Capacity Act. Utilizing an asset-based approach to social work, you will focus on the strengths and abilities of individuals to promote their well-being. You will complete Continuing Healthcare (CHC) checklists to determine eligibility for NHS continuing healthcare funding. Efficiently managing a demanding workload, you will ensure timely and effective support for clients. You will work closely with other healthcare professionals and agencies to provide comprehensive care and support. Additionally, you will conduct visits to clients in the hospital and their homes to assess needs and provide support.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
