This is an exciting opportunity to join the Hospital24 team as a Capacity & Flow Co-ordinator.
This role will require staff to work closely with a multi-professional team within the Medicine Division and wider Trust, with particular emphasis on working with clinicians taking part in the Hospital 24 rotation.
Hospital24 is a fast paced environment requiring the successful applicant to have the ability to think rationally and stay calm under pressure. To be successful in this role you will need to be able to juggle multiple tasks for a high volume of patients.
The role will provide cover to the department on both campus', in working hours, Monday-Friday.
The post holder is expected to highlight any potential delays in the patient’s journey to the appropriate teams.
The post holder will initiate internal escalation as per the Hospital 24 escalation triggers to the Senior Management Team. They will provide detailed and accurate information regarding the current situation and task load within Hospital 24, with an ability to convey complex information to senior colleagues.

The Department has operational responsibility for Hospital24 across the organisation. As such the post holder will be required to rotate between both main campuses that make up Nottingham University Hospitals NHS Trust.

Operational duties as below as well as administration tasks:
Monitor all clinical tasks, relating to capacity and flow, raised to Hospital 24 via NerveCentre and highlight any delays to appropriate clinical staff, advising on the instigation of local escalations
Monitor the allocation of clinical tasks, highlighting any delays and omissions to the appropriate clinical teams
Monitor the time in which clinical tasks are being completed. Where analysis of timescales shows there is a variation from Hospital 24 metrics and National Standards, the post holder will be responsible for escalating this to the individual clinician and if not resolved, to the senior management team
Monitor the reporting of time critical investigations (e.g. bloods, CT), directly liaising with the diagnostic departments to chase outstanding results. Significant delays will be highlighted to the appropriate teams
Show initiative in finding innovative solutions to problems, identifying bottlenecks and potential threats to Hospital 24 service delivery and Trust-wide flow and communicate to the senior team
Coordinate patient flow by identifying patients who are “medically safe” or “home today” with outstanding tasks and allocate these to the relevant Hospital 24 workforce
Provide and receive information orally, in writing or electronically to enable the ward teams to be kept informed of any pertinent issues affect the patient’s journey
Understand the Hospital 24 quality metrics (and the detail of how this is calculated). The Capacity and Flow Coordinator will work alongside the Bronze, Site Matron and Bed Management teams, as well as Hospital 24 clinical workforce teams, to expedite the patient’s journey and prioritise demand for the day
Link with relevant teams to ensure proactive preparation of TTOs for future discharges
Allocate clinical support to “medically safe” patients requiring a TTO regardless of their discharge date
