Job Title: Social Worker – Adult’s Single Point of Access Team
Location: Wrexham
Rate: £41.12 per hour
An exciting job role has recently become available at Wrexham. They are looking for a new member for their Adult’s Single Point of Access Team.
Responsibilities:
As a Social Worker in Wrexham’s Single Point of Access Team, you will be the first point of contact for referrals to Adult Social Care, handling enquiries from various professionals and individuals. You will assess and determine the most appropriate pathways for referrals, considering care and support needs or potential risks. Your role involves conducting initial assessments, engaging in What Matters conversations to identify personal outcomes, and providing information, advice, or signposting to other services. You will work alongside a multi-skilled team and be supported by an assistant Team Manager, with some opportunity for remote work after a probationary period.

Qualifications and Experience:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
