As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes and care homes. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence-based practice.
To support you in your new role we have developed an induction programme which allows you to learn the skills and knowledge required to be successful. The induction programme includes teaching sessions in the key areas of practice; Holistic Assessment and Care Planning, Palliative Care Awareness, Wound Care and Medication Administration. Along with your mentor you will work through the key areas of practice at your won pace until you feel confident and capable in undertaking visits yourself., Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Detailed job description and main responsibilities
As this is a community post, you must be able to travel to patients homes and clinic settings. Due to the rurality of Lincolnshire some areas are not well covered by public transport therefore having access to transport is essential.
The service works through the hours of 8:00am to 6:30pm 7 days a week and is covered by flexible working patterns.
The Community Nursing service delivers a 12 week induction programme for all new staff. We also offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential.
