Job Title: Social Worker – Adult East Locality Team
Location: Wolverhampton
Rate: Up to £33.20 per hour
Wolverhampton County Council is looking to take on an additional social worker for their Adult East Locality Team, working with adults aged 18+ with physical and learning disabilities and older adults.
What will your responsibilities be?
You must have experience within learning disabilities, community DOLs, and safeguarding. Conduct comprehensive assessments and develop personalized care plans for individuals and families in the community. Provide direct intervention and support services to address issues like mental health, substance abuse, and homelessness. Collaborate with multi-disciplinary teams and community partners, maintain accurate documentation, and evaluate interventions to ensure effective client outcomes.

Qualifications and Experiences:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
* Experience within learning disabilities, community DOLs, and safeguarding
