We are proud to be opening new supported living apartments in Ansdell, Lytham St Annes and we are looking for the right Team Leader to help us launch and manage this outstanding new service. This is an exciting opportunity for you to be part of this project, which will significantly enhance the lives of the People we Support and will be a great new place to work for our staff. This new service will set the standard moving forward for One Fylde. We will be supporting individuals with different levels of ability. Advanced Assisted Technology will bring further independence to the lives of the People we Support and all staff members will be fully trained in the use of this equipment. There will be 10 purpose-built apartments, with the benefit of communal areas and gardens in a beautiful property and a superb location. These apartments will provide more personal space, allow for more independence and increase confidence for each person living there. Also in the grounds will be a single tenancy bungalow. We will be working towards an almost paperless service so anyone applying should have good IT skills and be willing to learn new systems. As Team Leader, you will support the Registered Manager to drive this initiative forward. This more advanced Assisted Technology is new to One Fylde, but staff will be fully trained. As Team Leader you will ensure that staff are fully competent in the use of all new technology and equipment. Strong management and leadership skills will be key to ensuring the success of this new service. Open: a willingness to be honest and transparent Key responsibilities: Be responsible for promoting and safeguarding the welfare of those individuals they support Ensure all Supported Individuals have a support assessment prior to commencing service provision; ensure the people we support and their family or supporters fully participate in this. Ensure all the people we support have an agreed person-centred support plan and that all plans are reviewed at a frequency agreed with the Registered Manager. Ensure the people we support and their family or supporters are central to this. Undertake and record initial risk assessments for all new the people we support, including assessments in respect of support and their home environment. Complete regular risk assessment reviews at a frequency determined by the Registered Manager. In conjunction with the Registered Manager, agree a written risk management plan with the client in respect of each identified risk. Ensure Supported Living staff working with the person understands the identified risks and the corresponding risk management plan. In conjunction with the Registered Manager, ensure detailed plans are agreed with the people we support in respect of any support involving medicines and or personal finances. To ensure that staff rosters are completed in line with the commissioned levels of service; liaising with the Registered Manager to highlight discrepancies and concerns Compiling and updating people Planner Ensuring the people, we support receive information on events and activities in a timely manner. Maintain clear, accurate and timely records relating to all aspects of service delivery as required by the Registered Manager or the funding authority. Undertake regular client support shifts at a level agreed with the Registered Manager Complete regular service audits in line with organisational policy. In conjunction with the Registered Manager, conduct ongoing quality assurance activity with all the people we support in your house and use the feedback to improve the quality of the service. In conjunction with the Registered Manager, identify areas for development and improvement within the defined Registered of the service; contribute to the Continuous Improvement Plan Compile and produce management information in a timely manner e.g. reports as required by the Registered Manager Maintain up to date and accurate records relating to staff and the people we support using the relevant One Fylde recording system. Be familiar with the Care Quality Commission regulations and the Key Lines of Enquiry for inspection purposes. Contribute to planning for CQC inspections and take part in the inspection process. Adhere to One Fylde policies in respect of Data Protection and GDPR requirements at all times, ensuring the confidentiality and correct usage of personal data regarding staff and the people we support. Report any Safeguarding concerns following the organisation s Safeguarding policy and procedure.
