Holgate Primary Care Network (PCN) is looking to recruit a Care Coordinator to join our existing team working within our PCN Practices. The purpose of the role is to provide direct support to the PCN Practices in delivering and coordinating care to our patients with a focus on our Quality Outcome Framework (QoF), PCN DES & GP LIS requirements. This will include (but not limited to): Chronic Disease areas; Palliative Care; Learning Disabilities; Care Homes; Flu Vaccinations & Covid Booster Vaccinations and our new Urgent Care Hub.
Main duties of the job
Applicants are expected to have exceptional organisation and customer care skills, and empathy regarding patient needs. You will coordinate the care needs of individuals in line with the processes and requirements agreed by the PCN Management Team.You should have the ability to manage multiple tasks such as booking appointments, managing IT and remote booking systems, telephone skills, coordinating the care of housebound and care home residents, preparing clinics, record keeping and carrying out follow up calls with patients.Due to the nature of this role, it is desirable (but not essential) that applicants have previous experience of working in a health or medical environment. You will be required to undertake training as set out by the Personalised Care Institute.

Holgate Primary Care Network (PCN) is a network of three practices in Middlesbrough serving a population of approximately 45,000 patients. Holgate PCN is one of three PCNs in the Middlesbrough area and comprises Linthorpe Surgery, Borough Road & Nunthorpe Medical Group and Village Medical Centre. Two of the practices within the PCN are the two largest practices in the Middlesbrough locality.
Job responsibilities
Duties and responsibilities will include:Using GP systems to identify the cohorts of patients who require specific health checksCoordinating the booking of identified patients into the practice clinics (telephone or IT remote booking system)To regularly monitor the appointment systems to ensure all appointments are utilised and (working under the supervision of the Lead Care Coordinator) to take action to book all available appointmentsSupporting admin teams in practices to telephone patients that are due for a health check / review or have not attended a booked appointmentTo support practices in coordinating the vaccination of care home and housebound residents (including the preparation and coordination of patient lists, planning visits and communicating with patients)To review uptake of appointments and (with support from the Lead Care Coordinator) to identify at risk groups that need to be prioritised and contacted.Any other duties as requested by the Lead Care Coordinator or Management Team

Experience

* Ability to show empathy and be nonjudgmental toward distressed individuals.
* Experience working in an NHS setting

Qualifications
* GCSE grade A to C in English and Maths
* Qualified to NVQ level 2 in Health and Social Care (or similar)
* Qualified to NVQ level 3
