We are looking for a Practice Nurse to join our friendly practice. Based in Central Middlesbrough, we have a population of 7200 patients which is increasing all the time. As our Practice Nurse, you will be working as part of a team alongside another Practice Nurse and a HCA, both of whom have been with the practice a long time. The practice is high performing with QOF and LD reviews, as well as being one of the highest performing in the country for Health Heart Checks and has been rated highly in the Daily Mirror polls for appointments in the last 2 years running, being in the top 200 out of 6,500 practices.

Main Duties of the Job
Each day starts with daily audit time, allowing the Practice Nurse to check the rooms, equipment, and consumables to ensure everything is in place. Audit time is also given at the end of each clinic. Appointment times vary depending on the nature of the appointment. We do not have set clinics like a full session of childhood immunisations or chronic disease management; we do have set times from 8 am - 12 noon for a morning and 1 pm - end of shift time, including the daily audit times. We find the diversity keeps the sessions varied and fresh, ensuring you will never become bored and hopefully will feel excited to come to work daily.
The Nursing team works closely with the rest of the practice, and we are proud to say we are all part of a team that works together. The staff arrange regular team building activities for anyone to attend, including bowling, escape rooms, meals out, and even Bingo.
Stock checks are done monthly, and weekly checks are also performed. All audits are documented, and time is allowed for these.
We prefer candidates with experience in various areas of General Practice, including chronic disease management, childhood immunisations, and wound management. You will be expected to take bloods and perform ECGs. Do not let it deter you if you do not have everything on the desired list, as we will provide training to the right candidate.

We are a forward-thinking practice, and our team is fantastic; they all work together and help each other. The staff are appreciated, and we have a flexible approach to work. If anyone needs time off to attend an event, we will try to move clinics around, where possible, to save having to use annual leave.
The staff are award-winning and very friendly. Our appointment system is such that some days we even have spare appointments at the end of the day for the GPs. We can access many other services for patients, and our PCN runs its own Extended Access, allowing our staff the opportunity to work in the service if they want overtime.

Job Responsibilities
To respond to the general nursing requirements of practice patients.
Be responsible for meeting the nursing care needs of patients identified by the nurse or where care has been transferred to the nurse by the GP.
In accordance with the NMC, formulate individual care plans and evaluate outcomes at appropriate levels according to need.
To evaluate the outcome of that care, making changes to the care plan for individual patients where appropriate.
To demonstrate the rationale behind the various methods chosen to meet the care needs of patients, to evaluate those methods, and change them as necessary.
Review and plan care for patients with long-term conditions such as diabetes, asthma, and COPD.
Undertake cytology screening (on completion of training) with full knowledge of the national and local screening programmes and provide advice and follow-up when appropriate.
To promote a healthy lifestyle, including advice on diet and exercise, alcohol use, and to be aware of family history.
Provide full and accurate documentation of the nursing assessment of need and delivery of care.
Liaise with other members of the primary health care team and other agencies to ensure appropriate care is provided for the practice population.
