Employment Type: Full-Time/Permanent Job Summary: We are seeking a highly skilled and experienced Advanced Clinical Practitioner (ACP) with a specialised focus on medicines management. The successful candidate will play a pivotal role in optimising patient outcomes through expert clinical practice, education, and leadership in the use of medicines. They will work within the Four Pillars of Advanced Practice to provide excellent clinical care. The main duties will include using their own clinical competencies as part of a multi-disciplinary team and ICB to provide face to face and remote medications review, support management of patients with long-term conditions and co-morbidities, undertake medicines reconciliation, poly-pharmacy management, provide telephone consultation, repeat prescription standardisation, implementation and management, drug monitoring management, shared-care agreement management and clinical audits. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required. The role is pivotal to improving the quality of care and operational effectiveness within primary so requires, motivation, passion and resilience to deliver and excellent service within general practice. Key Responsibilities: Clinical Practice: • Conduct comprehensive patient assessments, including history taking and physical examinations. • Develop, implement, and evaluate individualised care plans with a strong emphasis on medicines management. • Prescribe and manage medications in accordance with clinical guidelines and best practices. • Monitor and evaluate the therapeutic effects of medications, adjusting treatment plans as necessary. • Provide expert advice on the safe and effective use of medicines to patients, families, and healthcare professionals. Medicines Management: • Lead and coordinate initiatives to optimise medicines use, reduce medication errors, and improve patient safety. • Develop and implement policies and procedures related to medicines management. • Conduct audits and participate in quality improvement projects to enhance medicines management practices. • Collaborate with pharmacy, nursing, and medical staff to ensure cohesive and comprehensive medication management. Education and Training: • Provide education and training to healthcare staff on medicines management and pharmacotherapy. • Mentor and support junior staff and students, fostering a culture of continuous learning and development. Leadership and Collaboration: • Participate in multidisciplinary team meetings and contribute to the development of clinical guidelines and pathways. • Represent the organisation at local, regional, and national forums related to medicines management. • Act as a resource and advocate for patients, families, and staff regarding medication-related issues. Qualifications: • Registered Healthcare Professional (e.g., Nurse, Pharmacist, or Allied Health Professional) with relevant post-registration qualifications. • Advanced Clinical Practitioner qualification (Master's level or equivalent). • Independent prescribing qualification. • Significant experience in a clinical role with a focus on medicines management. • Strong understanding of pharmacology, therapeutics, and clinical guidelines. • Excellent communication, leadership, and interpersonal skills. • Commitment to ongoing professional development and education.
