Are you a dedicated Social Worker with experience in supporting adults with learning disabilities? Newham Adult Services is looking for an experienced professional to join our Learning Disabilities Team, ensuring individuals receive person-centred care and support.
About the Role:
As a Social Worker in the Learning Disabilities Team, you will play a crucial role in assessing needs, developing support plans, and working with multi-agency partners to improve the well-being and independence of individuals with learning disabilities.
Key Responsibilities:
* Conduct assessments and reviews under the Care Act 2014.
* Develop care and support plans tailored to individual needs.
* Work collaboratively with service users, families, and health professionals.
* Advocate for individuals, ensuring their voices are heard in decision-making processes.
* Maintain accurate case records and adhere to safeguarding protocols.
* What We’re Looking For:
* Qualified Social Worker (Degree in Social Work, DipSW, CQSW, or equivalent) and registered with Social Work England.
* Experience working with adults with learning disabilities.
* Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.
* Excellent communication and assessment skills.
* Ability to work flexibly in a hybrid working environment.
