Community Nursing Sister/Charge Nurse
• Salary:£33,338 - £47,084 – Band 6

• Core Working Hours:Monday to Friday, with occasional weekend and late shifts as part of a rotational schedule.

In this role, you will provide support and leadership to a dedicated team that takes pride in delivering excellent care and prioritising the well-being of the community. Ideally, you will bring clinical experience in working with adults in a community setting.

We are looking for an enthusiastic and experienced nurse with a warm personality and excellent people skills. You should be eager to undertake relevant training, possess leadership experience, and demonstrate innovative practices. With strong management skills, you will take responsibility for managing a caseload of housebound patients with complex nursing needs in the community.
You will be responsible for delivering high-quality, evidence-based care to patients, their families, and carers within the community.
• Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
• Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
• Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
• Responsible for managing a defined caseload of patients, including the ongoing assessment of care needs and the development, implementation, and evaluation of care programs.
• Accountable for maintaining high standards of care and overseeing the activities of your team. This includes serving as a role model, sharing knowledge, and fostering the development of best practices.
