An opportunity has arisen for enthusiastic, motivated, hardworking individuals to join the Assertive Outreach (AO) Service. A RMN or Registered Mental Health Nurse qualification is essential.
The AO Service is a community based tertiary service who take referrals from CMHTs, Forensic Service, PIER and in-patient areas. We operate a team approach providing comprehensive care packages for people with severe and enduring mental health problems residing in Leicester, Leicestershire and Rutland.
Candidates will need to demonstrate a commitment to providing comprehensive care packages for people with enduring mental health problems within their own homes. Excellent assessment, risk management, organisational and communication skills are essential along with experience and a good working knowledge of the Care Programme Approach, Community mental health legislation and Hospital admission / discharge process. The ability to use your initiative and manage your time effectively is required along with the ability to work as part of a team and wider multi-agency services.

The working pattern for the post will be either Monday to Friday 9.00am to 5.00pm or Monday to Sunday 9.00am to 5.00pm.

Main duties of the job
The successful candidate will assess, plan, deliver and evaluate care for service users who are referred to the Assertive Outreach Service, including the assessment and management of risk, working within a psychosocial / family centred framework when assessing and planning individual packages of care. While being responsible for organising and planning your work as an autonomous practitioner managing your time effectively and liaising closely with other agencies and disciplines.
The post holder will undertake the role of Careco-ordinator/Lead professional for a case load of service users with complex mental health problems who are difficult to engage and require intensive packages of care, developing therapeutic relationships with service users in order to provide support and assistance. To administer and monitor the effects and adverse effects of prescribed medication and liaise with medical staff regarding any necessary adjustments in medication. To offer advice / information to clients and carers in regard to prescribed medications.
To possess enhanced communication skills to be able to communicate with service users and cares who are experiencing psychotic symptoms and may be in distress and to undertake the role of Lead Clinician for people who are detained under a Community Treatment Order completing formal tribunal/hospital managers reports for people detained under a section of the MHA or a Community treatment Order.
