We have an exciting opportunity for a self-motivated, experienced therapist to join our Urgent Care Response Team in Oldham Community.
We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries supporting the NHS Long Term Plan and admission avoidance agenda.
The post holder will work as part of the multi-disciplinary team, driving the changes to community care response to support the hospital avoidance agenda. The post holder will have the opportunity to work across the Enablement and Urgent Care services, including A&E Therapy Services and Intermediate Care bed base. They will work in line with the agreed strategic objectives of Northern Care Alliance.
The aim of the team is to reduce unnecessary hospital admissions and prevent avoidable Emergency Department attendances by providing holistic multidisciplinary intervention and person-centred approach support in order to stabilise patients in their own home or usual residence. The team also support hospital discharge and provide assessments in patients' own homes.

The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team. The candidate will be able to demonstrate strategic planning and have a strong clinical background in Urgent Care or a similar field. Excellent time management and advanced communication skills are required along with awareness and understanding of local, regional and national agendas in respect to Health & Social care. You must hold a UK driving license and have access to a vehicle. We are looking to recruit an experienced band 6 therapist.
