At HCRG care group we are dedicated to transforming healthcare. As a Health Visitor you will devise and implement personalised care plans for each child based on a needs assessment. You will cover the localities of Radstock, Midsomer Norton, Paulton and the surrounding areas.
You will join a welcoming and supportive team of Band 6 SCPHN Health Visitors, Band 5 Staff Nurses and Early Years Community Practitioners working to deliver Universal Childrens Services! We work in line with the Healthy Child Programme to improve health, reduce inequalities and safeguard children & young people.
We are dedicated to helping you improve your skills and progress in your career with great opportunities for course, apprenticeships as well as a ringfenced £100,000 a year used to implement the ideas of HCRG Care Group employees into practice to improve how we deliver healthcare!

Main duties of the job
As a Health Visitor you will:
1. Devise and implement individualised care plans for each child based on a needs assessment
2. Identify and assess health, growth and development in the 0-5 age range (0-19 Pathway) to advise and refer appropriately
3. To follow the guidance of the Unicef Baby Friendly Initiative across the organisation .
4. Provide information and education in support of Childhood Immunisations.
5. Participate in effective partnership working with other agencies, including Early Help Assessment (EHA) of childrens needs
6. Actively involve the child / parent / carer / family in the planning and implementation of any proposed programmes.
7. Identify children at risk and follow B&NES safeguarding policies and procedures.
8. Provide information and education in support of Childhood Immunisations.

As a Health Visitor, youll be part of our valued of Health Visiting Service team in BaNES at our Somer Valley Team.

The Ideal Candidate
Our Ideal Candidate will:
17. Hold NMC registration
18. Be a qualified SCPHN Health Visitor
19. Have a driving license and access to a car
20. Experience working as a health visitor or in a health visiting or childrens nursing team previously
We also welcome applications from newly qualified SCPHN Health Visitor students as well as students currently not yet completed their course.
