Community Psychiatric Nurse/Mental Health Practitioner

We are seeking an enthusiastic and motivated Band 6 Mental Health Practitioner to join our MHSOP Intensive Home Treatment Team within Gedling and Hucknall. We provide assessment, treatment, and support for service users of any age with a diagnosis of dementia or people over the age of 65 with a mental health condition within an 8 week time frame.
The team provides specialist assessment, therapy & treatment, working to an individually tailored care plan to adopt a flexible, patient-centred approach enabling the older person with mental health difficulties to engage in services available to them and to lead as independent a life as possible.
Delivered in people's own homes within the community, the service makes every effort to prevent unnecessary admission to hospital and facilitates the opportunity for people to return home following a hospital admission or period in residential care.
There will be an expectation that the team will work closely with current Community Mental Health Teams (CMHT) in the locality and with the CRHT's.
There will be an expectation that the Band 6 Clinician will be the duty worker for the team.
Main duties of the job
As a Band 6 Mental Health Practitioner your main duties will be:
* You will be an experienced Registered Mental Health Nurse or Occupational Therapist and have current NMC/HCPC registration.
* You will complete both triage assessments and more detailed initial assessments of new referrals. You will also be expected to provide support via the teams duty system, either from telephone support or face to face.
* This is a leadership role and you will be expected to develop specialist skills in working with patients with high levels of acuity and increasing complex mental health condition and/or diagnosis of dementia. You will also be expected to support students and more junior staff within the team through clinical supervision and mentorship, and you may be asked to line manage some staff.
* We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, have a sound knowledge of risk assessment and be able to demonstrate sound evidence-based practice. You will have excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.

Job responsibilities
Day to Day Duties:
* Caseload management for patients living in the community, and acting as care coordinator with the Intensive Home Treatment Team.
* Triage of referrals, provision of initial assessments and one-to-one interventions with service users.
* To undertake and record comprehensive need/risk assessments, devise and implement treatment plans for service users.
* To demonstrate the application of specialist skills and knowledge in order to maintain professional competence and fitness to practice as a nurse practitioner.
* To establish and maintain robust communication networks with clients, carers, team members, other health care and social care workers within urgent care and the community.
* To use interpersonal skills in diffusing potentially emotive situations while undertaking assessments and pathway planning within acute settings.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Person Specification
Qualifications
* Professional Qualification (Mental Health) to degree level or equivalent
* Maintains current registration with the NMC or equivalent registration.
* Further specialist knowledge/post registration experience in specific service area or other formal training
Knowledge
* Knowledge and understanding of current legislation and understanding of clinical risk assessments
* Knowledge of the nursing 6 c's
* Awareness of national and local issues impacting on nursing care
* Knowledge of Management theories
Skills
* Ability to work as part of a team
* Ability to be flexible and handle conflict issues
* Good skills in assessment intervention and evaluation of patients nursing care
* Clinical supervision skills
Experience
* Demonstrates satisfactory clinical expertise for the banding
