This will include conducting new patient assessments into the service, acting as named nurse for clients under CPA, being instrumental in involving clients in planning and evaluating their treatment.
The post holder will liaise with other services or professionals as necessary, co-ordinating the effective transfer of in-patients to the community team, and undertake group work as appropriate. The post holder will be expected to maintain excellent documentation/record keeping and correspondence, and work as part of the MDT team to achieve the highest possible care for the client group. Liaising with clients’ families and carers as appropriate and to carefully plan discharges and aftercare is also essential.
