Clinical Nurse Specialist - CYP Eating Disorders
NELFT North East London Foundation Trust
Band 7- Clinical Nurse Specialist in Eating Disorder (CYP)
We are looking for a Specialist Nurse to join our well-established, innovative Nurse-led CYP ED Intensive community treatment service. This is a great opportunity to join our highly skilled, dedicated, and friendly team.
NELFT CYP Intensive community treatment service (referred to as HIVE)
The HIVE treatment model provides intensive community treatment including therapeutic and physical health support to CYP and families within the family home as an alternative to hospital admission. The HIVE team also supports admissions and facilitates safe and early discharges from inpatient/hospital care when admission is clinically indicated.
The HIVE service is an 8-week programme for young people open to the Eating Disorder Service who have struggled to progress with routine treatment and require more intensive intervention to prevent further deterioration, a protracted illness, and/or hospital admission. Young People and their caregivers are offered needs-led, intensive community support using a blended model of F2F and virtual appointments in a variety of non-hospital settings including our clinic and family homes. Programmes are individualised and include a variety of supported meals, individual work with the YP, ongoing FBT, parent work, and close physical health monitoring.
Main duties of the job
As an integral part of the HIVE team, you will be part of an MDT providing:
1. In-reach to the local GAU, supporting the YPs journey from community to inpatient, maintaining clinical contact with care continuity being a key principle of the service.
2. Support established pathways with paediatric inpatient teams for referring patients ensuring appropriate care is provided from acute medical admission to discharge.
3. Enable a greater physical presence on the paediatric wards by HIVE staff.
4. Support hospital colleagues to implement ED specific care plan, especially in complex cases with co-morbid presentations.
5. Provide comprehensive handovers to ward staff, sharing of risk assessments/reports and formulation, meal support on the ward including discussion at the weekly liaison meetings for the duration of the YP stay.
6. Join ward round/weekly MDT discussions to support continuity of care for SEDU's & GAU as required.
7. Attend weekly bed management meeting as required.
