Please Note: Interviews for this post will take place on Monday 3rd February 2026 The Role An exciting opportunity has arisen to in Bolton to join the Transition team working with young people aged 14-25 with special eduction needs and disabilites. The successful candidate will work with both Children and Adult services to ensure a smooth transition for young people in preparing them for adulthood. This role will be responsible for discharging duties in accordance with the Care Act 2014. Undertaking holistic assessments and developing transition plans that will inform the appropriate future pathway and support for the young person. Furthermore, identifying and providing carers assessments. About You We are looking to recruit a qualified social worker, who is enthusiastic and motivated with an interest and passion for working with young people with disabilities, understanding their needs, current and future aspirations, and to support them in preparing for adulthood. You will have the ability to communicate, develop, maintain and improve relationships working in collaboration with young people, their families and key partners. You will work with commissioners alongside the Deputy/Team manager to develop the market and ensure there is sufficient, effective and good quality provision in the local area.
