DCHS has a fabulous opportunity for a SCPHN School Nurse to join the Children's Community Services Team on a full-time basis (37.5 hours per week).

Working with the skill mix team, you will deliver the Healthy Child Programme universally along with progressive levels of service and safeguarding children according to identified need.
You will provide services in partnership with the local voluntary sector and multiagency teams and participate in service improvement both within the locality and across the county.

Main duties of the job
You will manage and develop the Children's Community Services Team (5-19) and contribute to the development of services to deliver the commissioned service specification.
You will be part of the multi-disciplinary team including the wider local children's workforce, working within a public health framework, to meet the prioritised health needs of children and families.
You will have responsibility for delivery of the key performance indicators for a community of school aged children within an identified specific action plan.
You will undertake responsibility as 'named nurse' for a Multi-Agency Team area and inclusive schools within a specified locality.
You will carry continuing responsibility for the assessment of health needs and the development, implementation and evaluation of programmes of intervention. To create and promote a positive learning environment for junior staff and students on placement.
Whilst in this post, you will have an office base at Newholme Hospital and cover schools primarily in the Buxton area. You will be required however to travel around the locality, which stretches from Matlock to Glossop, as part of the 5-19 public health team.
Travel to other parts of Derbyshire is necessary on occasion, for example to attend meetings and training sessions.
Job responsibilities
* Line management of the Childrens Community Services team (5-19) within a defined area in order to assist in the leadership, management and co-ordination of the delivery of the Healthy Child Programme and commissioned service specification.
* Assess and determine the health needs of the school age children and wider population to ensure that the services delivered meet the needs of that of the population.
* Develop an agreed specialist health interest and become the named champion for this practice area. e.g. Public Health initiatives/ interventions and provide advice/information as necessary.
* Identify, plan and facilitate the delivery of appropriate health education/promotion programmes to meet the health needs of the defined population in line with national and local priorities and strategies.
* Facilitate the delivery of services under the direction of the healthy school's environment programme.
* Contribute to the planning and organisation of workplace induction of new staff and support and facilitate the continuous professional and practices development of staff within the children's services team.
* Establish and develop multi-agency/multi professional working relationships in line with national and local priorities and strategies.
* Responsible for child safeguarding issues in own identified area and support other children's services staff in relation to child protection issues.
* Support and enhance the learning environment for pre and post registered students and other health learners.
* Responsible for the assessment and teaching of pre-registration students and return to practice students to facilitate learning and understanding of children's services to meet the needs of the student and educational establishment or Trust.
* Assist in the co-ordination/planning and delivery of immunisation programmes as required, ensuring the staff within the team follow procedures. Develop and maintain appropriate skills to effectively deliver the programme.
* Assist in the monitoring of hours worked for staff within the team via e-rostering to ensure that adequate staffing levels are maintained to ensure a quality service is delivered across the localities.
* Assist in the collation, delivery and presentation of statistical information and data as required to meet the NHS and local children's services agendas.
* Comply with Trust policies and procedures.
* Develop and implement clinical supervision/reflective practice and continuous learning both for skill mix staff and oneself.
* Undertake staff appraisal, taking into account both the needs of the service and the individual practitioner to encourage personal and professional development of staff and self in order to maintain and improve knowledge and skills in accordance with NMC guidelines. To share good practice with colleagues.
* Identify, assess and determine client need and where appropriate prescribe from within the nurse prescribing formulary in accordance with evidence practice based, patient medical history and Trust Policies and Procedures.
* Promote Clinical Governance Guidelines and be proactive in the development of a culture that is committed to innovation and quality improvement through the use of research and audit to enable services to be continuously improved, to include clinical benchmarking, professional development, competence frameworks and clinical supervision.
