An exciting opportunity has arisen for an enthusiastic and hardworking individual to become part of the Dietetic Team in Powys, as a Dietetic Support Worker. You will be part of a new project to develop Nutritional Pathways for people who are living with Frailty.
Following appropriate training, your role would support the delivery of high-quality nutritional care to patients with frailty as a part of the Dietetic Team. Under supervision, you will support patients to meet their nutritional needs by developing and providing care plans with a focus on 'Food First' or other nutrition related issues. Other roles may include group education sessions to nursing and care staff with some administrative responsibilities. Opportunities to be part of audit and to work closely with the wider multidisciplinary team is encouraged.

Main duties of the job
1. To deliver community-based nutrition intervention for service users in a variety of settings under the direction of appropriate senior qualified staff.
2. To nutritionally assess and provide nutritional care plans to patients in a variety of settings under the direction of appropriate senior qualified staff.
3. To manage telephone calls appropriately and relay messages accurately and in a timely fashion.
4. To represent the Dietitians at events and meetings as appropriate.
5. To participate at team meetings.
6. To work safely and appropriately in an unsupervised capacity as part of the Dietetic team.
7. To undertake activities alone within the community setting in accordance with the Lone Worker Policy.
