We are looking for an experienced Band 3 HCA to cover 4 night shifts per week within the District Nursing Out Of Hours Service. The role includes caring for patients who are approaching the end of life. It involves working on a one to one basis ie spending a full shift with one patient, in their own home. You will be working without close supervision, and will therefore need to use your own initiative, whilst following the patient's care plan. You must also be passionate about making a difference to people’s lives, helping them to live as well as possible and to die with dignity. As well as being able to work independently, you will possess good time management and organizational skills and be expected to be innovative and flexible to work in other areas - if required - to meet service need. We will expect you to have a talent for forging strong empathetic relationships with a variety of people, whilst providing palliative care. We will of course provide excellent on-going training and support. Experience of caring for people at the end of life is essential. Your role will include delivering a range of nursing interventions to patients in their own homes. These may include monitoring symptoms, undertaking simple dressing and continence care, and keeping detailed records. You will also help patients to meet their hygiene and dietary needs. You will be adaptable, reliable and approachable, and will be expected to support your peers, the team leader and the organisation in promoting the Trusts Values and Behaviors. Please note whilst your main base will be at Crewe you will be required to work across the footprint of Central Cheshire Integrated Care Partnership.
