We are seeking a highly motivated Clinical Pharmacist to join our team in North Liverpool PCN. As a Network Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications, providing pharmaceutical care and expertise to patients, and collaborating with healthcare professionals to deliver exceptional patient outcomes. North Liverpool is the largest PCN in Liverpool with 21 practices and a weighted population size of approximately 150,000 patients.
Main duties of the job
Responsibilities
1. Conduct comprehensive medication reviews and assessments to identify and resolve medication-related problems for patients.
2. Collaborate with physicians and other healthcare professionals to develop and implement personalised medication therapy plans.
3. Provide patient education on medication use, potential side effects, and drug interactions.
4. Monitor and adjust medication therapy based on patient response and laboratory values.
5. Conduct medication reconciliation and identify discrepancies to ensure accurate and up-to-date medication lists.
6. Participate in multidisciplinary meetings to discuss patient care and optimise treatment outcomes.
7. Provide education sessions for GPs, practice staff, other health professionals, and patient groups.
8. Stay up to date with current literature and guidelines to provide evidence-based pharmaceutical care.

We have a large pharmacy team consisting of 14 pharmacists and 9 technicians. We offer supportive induction training to equip you with the skills and knowledge you need to become an effective member of the team. We also offer training and education to support you to develop further.
Person Specification
Qualifications
* Master of Pharmacy degree from an accredited institution.
* Active pharmacy registration with GPhC.
* Good knowledge of pharmacology, therapeutics, and medication management.
* Excellent communication and interpersonal skills.
* Ability to work well in a fast-paced environment and handle multiple priorities.
* Non-medical Prescribing qualification desirable or an interest in working towards obtaining the qualification.
Experience
* Experience of working in a team.
* Experience of Windows Based Computer Systems.
* Experience of the EMIS clinical system.
* Experience in promoting Health Improvement Campaigns.
* Experience within a GP setting and knowledge of the procedures.
