Main duties of the job
This is an exciting opportunity to be part of an expanding team which prides itself on high quality patient care to the patients registered at The Wall House Surgery, a friendly training practice looking after 22,000 patients. The post holder will provide advice for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients being treated.
The post holder will demonstrate critical thinking in the decision-making process. Under the supervision of the duty GP (usually a GP Partner), as part of the acute team, they will work collaboratively with the wider primary care team and provide advice to meet the needs of patients, and their relatives/carers, while at all times working within the agreed policies and procedures supporting the way care is provided.
The post holder will be an experienced Nurse Practitioner who holds an appropriate, specific, advanced professional qualification which meets NMC requirements for high-level practice.

Job description
Job responsibilities
Assess and treat patientsindependently as part of the acute team. Patients will be triaged by a seniorGP to ensure appropriate for the competencies of the ANP.
Manage patients presenting with arange of acute and chronic medical conditions, providing subject matter expertadvice to patients
Implement and evaluate individualspecialised treatment plans for acute presentations of chronic disease patientswith GP supervision where required
Identify, manage and support patientsas risk of developing long-term conditions, preventing adverse effects on thepatients health
Provide advanced, specialist nursingcare to patients as required in accordance with clinical based evidence, NICEand the NSF
Undertake the collection ofpathological specimens as appropriate
Request pathology services asnecessary
Process and interpret pathology andother test results as required
Maintain accurate clinical records inconjunction with extant legislation
In addition to the primaryresponsibilities, the advanced nurse practitioner may be requested to:
Participate in local initiatives to enhance service delivery and patientcare
Support and participate in shared learning within the practice includinghaving students / other learners to shadow clinics.
Develop an area of specialist interest, taking the lead within thepractice. Where there is a business need we would be keen to support you indeveloping professional special interests. We have a wide range of GPs withadditional special interests including minor surgery, frailty, education,menopause, contraception and womens health and palliative care among others andwe would be happy to support and mentor you in developing interests.
Continually review clinical practices, responding to national policiesand initiatives where appropriate
Participate in the review of significant and near-miss events applying astructured approach root cause analysis (RCA)
Drives the development of acute care services within the practice,liaising with external agencies and professional organisations as required
Develops practice administrative and clinical protocols in line with theneeds of the patient and current legislation
We have a nurse manager and at thistime would not be looking for you to lead the nursing team. Your role would sitwithin the acute team primarily working as part of a team supporting the acutecare of our patients (with the potential for future development of interests tobroaden the scope).

Essential
1. Registered Nurse
2. Post graduate diploma or degree (Advanced Practice Qualification)
3. Qualified Nurse Prescriber
4. BLS / ALS
5. Demonstrate patient assessment skills
6. Experience of supporting GP's in clinical decision making
7. Ability to work autonomously within the scope of practice
8. Experience of working within a similar role
9. Detailed knowledge of maintaining confidentiality and implementing data protection legislation requirements
10. Ability to work well as part of a team
11. Positive approach and calm under pressure
12. Excellent verbal and written communication
