You will be working for Shropshire Community Health NHS Trust with cross-site working at The Royal Shrewsbury Hospital and The Princess Royal Hospital, Telford.
This is an exciting opportunity to make a difference to the care of patients enabling them to step down to their normal place of residence faster.

Main duties of the job
Working within the ward environment as part of the multidisciplinary team, provide a continuous coordinated approach to the safe and timely discharge of patients with simple and complex discharge needs.
Communicate and liaise with patients, their families and carers and other key external stakeholders who play an integral role in patient discharge.
Support the delivery of the SAFER red to green approach to expediting patient care.
The role requires a high proportion of autonomous working within the ward team.

Person specification
Experience
* Experience of working within a hospital ward environment
* Experience of participating in the patient discharge process
* Working with hospital computer systems
Knowledge & Skills
* Knowledge of the patient discharge process
* Knowledge of the Red to Green concept for discharge
* Articulate with well-developed communication (verbal and written) and interpersonal skills.
* Good organisational skills; self-starter and able to use initiative appropriately with minimal supervision.
Training & Qualifications
* GCSE level maths and English or equivalent or level 3 NVQ in health care related topic
Other
* Ability to meet the travel needs of the post
