The successful candidate will be responsible for undertaking clinical assessment of adults' physical status pre-operatively and through collaborative working with clinical, nursing, and therapies teams to ensure adults are physically optimised for their scheduled surgery and the resources required to meet their individual psychological and care requirements are in place for their admission and discharge in conjunction with their family and the multi-disciplinary team.
We are looking for a motivated and enthusiastic Registered Band 6 Nurse to join our team. This post will play a crucial role in ensuring quality and safe care is maintained, whilst moving forward to meet the evolving needs of the service and population we serve.
Main duties of the job
The main duties of the role are to ensure that the patient receives a high standard of care in the department.
You Will Need
* An understanding of the pivotal role Pre Assessment has in the inpatient pathway.
* Good IT skills.
* Ability to work on own initiative and as part of a team.
* Able to utilise a variety of strategies to communicate effectively with adults and those involved in their care.
* Able to undertake clinical assessments utilising agreed clinical decision-making protocols to make appropriate recommendations regarding the patient's medical fitness for surgery, including suspending or removing patients from the waiting list and referring back to their GP.
* To commence discharge planning to ensure a smooth seamless pathway of care for adults.
* Undertake the triaging of patients recording their risk stratification.

Job Description
Job Responsibilities
* Deliver the provision of high quality pre-operative patient care, ensuring that the service continually meets the needs of patients, relatives, carers, and clinical colleagues.
* Regularly take charge of the department.
* Evaluate service delivery, identify areas for improvement and initiate change in conjunction with the Senior Sister and Divisional Head of Nursing and clinical colleagues.
* To communicate and work in a collaborative and co-operative manner within the multidisciplinary team, both within and externally of the Trust.
* To ensure a well-informed, safe admission to hospital for any given patient, providing individual patient care.
* To act as the patients' advocate and ensure specific standards of service are delivered and maintained.
* Acts as an effective role model.
* Ensure that risk management systems are adhered to and that untoward incidents are reported according to Trust Policy.
* Ensure that your own knowledge and expertise is maintained by participating in the in-house training programme, CPD, and Appraisal.
Person Specification
Skills

Knowledge, Training & Experience
* Registered Nurse.
* BSc Nursing or working towards.
* Mentorship Teaching and Assessing qualification.
