Job summary
To deliver nursing care in accordance with national and local policy and guidance and in accordance with the Nursing and Midwifery Council.
The post holder will be a Registered Nurse in the community 'community nurse' who works as part of a team to deliver nursing care and treatment autonomously as delegated by and under the direction of the District Nurse.
Main duties of the job
o The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.
o The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working.
o The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

We recognise that a highly skilled community nursing workforce is the cornerstone to safe patient care and avoiding unnecessary admission to hospital and have lots training and development opportunities available, alongside a career development pathway with opportunities for development at every level through to band 3 to band 8.
All community nurses work as part of a local team, closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses and use the electronic clinical record, EMIS with a work Ipad and Iphone provided.

We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. On achieving all relevant competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post.
Post holders will be available to work Monday to Sunday between 8 am and 8 pm and be able to travel to patients homes frequently throughout the day across the locality and across Guildford and Waverley.

Qualifications
Essential
1. Registered Level 1 Nurse with current Nursing and Midwifery Council registration
2. Educated to diploma or level 6 / degree level in relevant area
3. Supervisor / Mentorship qualification (within the first 2 years of post)
4. Evidence of continuing professional development
5. Area of special interest in the management and treatment of long-term conditions and willing to undertake additional education in this area
Knowledge
Essential
6. Knowledge of recent developments within health and social care
7. keeps up to date with evidence-based practice and research related to nursing and health care in the community
8. Ability to safely and effectively operate community equipment
9. Ability to assess nursing needs and implement nursing care and treatment
10. Ability to identify and pass on urgent client information to the District Nurse (or deputy senior community nurse)
11. Ability to work across a seven-day week between 8 am and 8 pm as per service requirements
12. Ability to prioritise visits in accordance with patient clinical need
13. Ability to organise own workload without direct supervision
14. Ability to teach and supervise students and team colleagues
15. Able to teach patients and ensure they are supported with health promotion and self care
16. Dexterity to enable clinical skills to be performed removal of sutures venepuncture IV Therapy
17. Able to use databases and other IT programmes, including MicroSoft Word, Excel, Powerpoint, Emails, Virtual Meeting platforms
18. Ability to input and navigate around the community electronic patient record system. EMIS (training will be provided)
