The Enhance Care Clinical Specialist Nurse will develop strong professional relationships with the Trust, MHLT/CCATT and key clinicians in HPFT to ensure good lines of communication between the Trust and the mental health practitioners. Act as a key point of contact and liaison for HPFT staff for operational and clinical delivery of mental health care for patient within WHTH. Demonstrate excellent communication skills across agencies. To work closely with HPFT, mental health commissioners and the wider health system to ensure best care for patients with mental health and complex social need. To generate and sustain relationships that promote dignity, diversity, rights and responsibilities across mental health and wider networks. Communicate regularly with the Trusts Mental Health Matron regarding operational team to ensure mental health / complex needs patients are in appropriate beds and ensure good patient flow across the Trust. Communicate and escalate to the Trust Mental Health Matron any issues with bed management / patient flow involving mental health / complex needs patients. Attend all relevant MDT meetings for mental health patients on behalf of the trust to ensure delivery of best care and treatment in the most appropriate setting ensuring the patients needs and wishes are considered. Work towards ensuring clinicians treat patients holistically considering both physical and mental health needs and that these are often interlinked. Communicate and work across the wider health and care systems and ICS to ensure best outcomes for patients experiencing mental health crisis. The Enhance Care Clinical Specialist Nurse will advocate on behalf of all patients.
