Main duties of the job
Coordination of team activities to ensure that the NHS All Age Continuing Care processes run efficiently and within mandated timescales.
Act as the first point of contact for internal and external stakeholders, ensuring queries are directed appropriately.
Monitor the commissioning team inbox, ensuring prompt responses to enquiries and efficient allocation and escalation.
Support the team with the management of projects, gathering information and undertaking enquiries as necessary.
Assist in collating and preparing data for provider performance reviews and audits.
Schedule, organise, and minute meetings, ensuring follow-up actions are assigned and tracked.
Support the team with documentation for contract setup, renewals, and compliance monitoring.
Communicate daily, both verbally and in writing, with team colleagues (clinical and non-clinical), care providers, and other health and social care agencies, providing and receiving sensitive information.
Develop good working relationships with care providers, Local Authority colleagues, and colleagues from other NHS bodies.
