We are looking for a Senior Practitioner for Bury Town Adult Social Care Team. This is an opportunity to develop your skills and enhance your knowledge within the team. It will enable you to progress in your role and develop key skills to enhance your practice and future direction. This is a key role within the organisation, the post holder will support and liaise with our Adult Social Care practitioners within the Bury Town Integrated Team (INT), this will ensure that the highest standards of health, social care and reablement continues to be delivered within the locality. The main purpose of the role is to support and work with our Adult Social Care practitioners and support the manager within the team. You will work closely with INT and contribute to the effective leadership of the team. You will also support and liaise with the Locality Lead in delivering effective integration between health and social care through collaboration with stakeholders and partners across the system to deliver better outcomes for the people of Suffolk. Working alongside all key partners, will be an important part of the role, ensuring the health and social care systems are joined up and accessible. You'll work effectively with complex social situations which include Self- Neglect and Hoarding Safeguarding’s and Court of Protection paperwork and a variety of other duties within your role. You will supervise a small number of staff and ensure they are meeting their Performance Development Review objectives which have been set out. Your role and responsibilities work with a range of customers, helping them maintain or regain their independence by drawing on their strengths, networks, and the local community using the Signs of Safety (SOS) model play a critical role in managing the demand for adult social care, including using digital solutions as tools for assessment and care collaborate closely with health colleagues as part of Health and Social Care Integration (INT) focus on achieving the best outcomes for people, championing independence with a positive approach to appropriate risk-taking conduct asset-based assessments and reviews, some of which may be complex and challenging perform background research on referrals, which can be distressing and emotionally/mentally demanding serve as a role model, demonstrating to unqualified team members how to manage creatively within budgetary restraints to achieve value for money for customers, carers, and the service provide sound financial and benefit advice in complex and conflicting situations promote a culture where individuals and communities can exercise their rights to autonomy and self-determination, balancing this effectively with safeguarding responsibilities handle living equipment and adaptations, including more complex solutions such as adaptations for social housing, and champion the use of Assistive Technology including Digital Care solutions appropriately identify eligibility in line with current legislation and policy advise customers and carers on their rights, the options available to them, and how they can be better supported in difficult circumstances implement and review Short Term Enablement Plans, identify Personal Budgets, and implement and review Support Plans with customers act as a Practice/Policy Champion within the team, advising team members and contributing to their development.
