Summary The West Ridge Center Caseworker serves as part of the West Ridge Center Team and collaboratively works to match West Ridge Center patients with the necessary resources to improve their health and well-being. This role plays an integral part in identifying needs and available resources, promoting, and assisting with enrollment, and follow through on acceptance/participation in programs. There is an educational component to this role for both the patients being served as well as sharing of knowledge and experience with others who support these programs and initiatives. Minimum Education Bachelors’ degree in Human Services field or equivalent combination of education and experience. Minimum Work Experience 1 year experience in community outreach or support services preferred. Required Skills, Knowledge, and Abilities Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds. Demonstrated knowledge of community resources. Excellent oral and written communication skills. Demonstrated ability to convey information via accurate documentation. Ability to present information in a clear and comprehensive manner. Customer service skills. Demonstrated strong knowledge of basic computer skills. Salary Range: $47,500.00 - $75,800.00 PM24
