Job Title: Social Worker

Rate: £33.30 per hour

The Wolverhampton City Council is looking for a Social Worker to join their Learning Disabilities team, offering a pay rate of £35 per hour.
What will your responsibilities be?
In this role, you will work with young adults aged 17.5 to 25 years who have varying levels of SEND. You will implement the 3 Conversation Model to support these young adults in their transition. You will arrange care and support in the community, including college provisions, day opportunities, and supported living placements. You will also undertake Mental Capacity Assessments and apply the findings within the 3 Conversation Model. You will complete Deprivation of Liberty Standards applications. Additionally, you will collaborate closely with health and education professionals to ensure that plans are fair and meet the identified needs.

Qualifications and Experiences:
* Social Work England registration.
* Eligible to work in the UK.
* Hold a full UK licence.
* Degree level or equivalent in Social Work.
* Knowledge of the 3 Conversation Model.
* Experience working with young adults with SEND.
