Job Title: Social Worker - Grade K Location: Derbyshire Rate: £29 per hour This position is part of the Preparing for Adulthood Team, with a primary focus on conducting a review project. What will your responsibilities be? In this role, you will conduct thorough reviews and assessments to support young adults transitioning to adulthood. You will collaborate with team members to develop and implement effective support strategies, while maintaining accurate and up-to-date records of all casework and interventions. Engaging with clients, families, and other stakeholders to gather information and provide necessary support is essential. You will ensure compliance with relevant policies, procedures, and regulations, and participate in team meetings, training sessions, and professional development opportunities. Additionally, you will provide guidance and support tailored to the individual needs and goals of clients, and liaise with external agencies and organizations to coordinate services and resources for clients.
