The Learning Disability Liaison Nurse will contribute to the robust Learning Disability patient identification and flagging, provision of advice and practical support regarding reasonable adjustments, provision of training, collaborative interagency working and contributing to governance, audit and service development in relation to patients with a learning disability.
The Learning Disability Liaison Nurse will work in partnership with clients, their families / carers, and health/social care professionals, to provide a specialist service for people with learning disabilities accessing Northern Care Alliance Hospital Services.
The Learning Disability Liaison Nursing Team provide support to both Adults and Children with Learning Disabilities. This specific post will support patients of all ages.
The post holder will
· Effectively and independently manage a caseload of work reflective of skills and experience.
· Provide specialist advice to support the identification of required reasonable adjustments and care/treatment plans incorporating Person Centred Planning/Strengths Based approaches.
· Work collaboratively with patients, families, provider services all members of the MDT (Multi-Disciplinary Team).
· Contribute to the assessment and planning of support for children, young people and adults with Learning Disabilities.
· Contribute to service development for all patients and a specific focus on service development for children and young people.
· Provide specialist reports as required by the service. This may include reports for the Coroner and Court of Protection.
· Maintain accurate and legible records in accordance with Northern Care Alliance and national professional policies and guidelines.
· Practice in accordance with the professional, ethical and legal framework for nursing.
· Carry out clinical procedures in line with professional requirements.
· Play a key role in the supervision and assessment of all learners and junior staff, providing a teaching role and acting as a named supervisor/assessor.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.
