Special Educational Needs and Disabilities (SEND) Health Coordinator
We are seeking two qualified, clinically registered professionals to join our developing SEND Health Team as SEND Health Coordinators. This is a fantastic opportunity for experienced nurses or allied health professionals with experience of working with children and young people with Special Educational Needs and Disabilities (SEND).
As part of the West Yorkshire Integrated Care Board (WYICB - Leeds Place), you will play a key role in helping us deliver on the statutory responsibilities set out in the Children and Families Act (2014) and the SEND Code of Practice (2014). You will be supporting children and young people aged 0-25 with SEND to ensure they receive the care and support they need to thrive.
We welcome both full-time and part-time applicants.
In this role, you will support the coordination of high-quality and timely health advice for Education, Health, and Care Plans (EHCPs). You will play a key role in promoting the importance of health within the SEND framework, offering guidance and support for EHC assessments, Annual Reviews, and contributing to multi-agency panels.
The successful candidate will collaborate closely with key system partners, including health, education, local authorities, as well as children and young people with SEND and their families/carers.
Main duties of the job
The post holder will lead and co-ordinate the contribution of high-quality and timely health advice to Education Health and Care Plans (EHCPs) and to promote the role of health in SEND.
The post holder will:
1. Offer guidance and support to health providers and the wider health and care partnership, children and young people/parent carers to ensure effective and timely contribution of Specialist Health Advice to EHCPs.
2. Attend multi-agency panels, support training and workforce development in relation to health's role in SEND and EHCPs, support mediation and tribunals, and contribute to quality assurance via delivery of Health's EHCP Quality Assurance Framework.
The post holder will form part of the health SEND team, working closely with the Clinical Lead SEND and the DCO- SEND and will be part of our Leeds Health and Care Partnership where we work together with our partners and the people of Leeds with a commitment to share resources, ideas and best practice to improve health outcomes and reduce health inequalities across the city.
The post holder will work flexibly to meet the needs of our system. We are proud to be part of the West Yorkshire Health and Care Partnership, and you will be expected to contribute, and sometimes lead, on work across the West Yorkshire geography.
