The postholder will support the registered nursing team in providing care and support to our patients on the Inpatient Unit at our Hospice.
This role offers excellent opportunity for you to develop and enhance your healthcare skills and experience within palliative and end of life care, working within our Inpatient Unit.
Part-time position of 22.5 hours per week - to be worked flexibly including weekends, bank holidays, evenings/nights
Main duties of the job
About the role:
You will deliver high quality Specialist Palliative and End of Life care as part of the Inpatient Unit multidisciplinary team, under the direction of the designated nursing team.
You will support the assessment, planning and evaluation of individualised patient centered care, ensuring a holistic approach that is sensitive and responsive to the patients' ever-changing needs, maintaining the patients comfort, dignity and privacy at all times.
You will ensure all aspects of the care provided are in accordance with Mountbatten's Values and Care Standards.
About you:
You will be qualified to Level 2 Health and Social Care Certificate and have previous care experience. The role will be part of a dedicated team of professionals, who will also be able to work under their own initiative to ensure the highest standard of care. A can-do attitude with strong communication skills will be required.
You will be required to deliver a high standard of direct patient care, assisting the patient in all activities of daily living, with both direct and indirect supervision as delegated by the qualified nurse.
Job responsibilities
· Promote a patient centred approach to care delivery by assisting patients in all daily living activities, as required by the nursing team and the patients care requirements.
· Ensure patient comfort, dignity and privacy is maintained at all times and report any concerns to the nursing team.
· Assist in the monitoring of patients’ blood pressure, temperature, pulse, respiratory rate, peak flow and oxygen saturation on completion of the internal training day and competency pack.
· Obtain specimens of body fluids as requested by the qualified nurse on completion of the internal training day and competency pack.
· Assist in emergency situations, following appropriate training.
· Escort patients to hospital and other appointments.
· Assist in the reception of patients, their relatives and general ward/department visitors.
· Report any change in condition of patients to a registered nurse.
· Order meals and liaise with kitchen.
· Plan, organise, prioritise and control work, ensuring effective use of time and financial resources.
Person Specification
Qualifications
* Educated to a good standard
* Care Certificate or NVQ level 2 in health care or equivalent or willing to study towards
* Specialist palliative care training
Skills
* Demonstrate commitment to the patients right to be involved in decision making relating to their health care and ensuring decisions are understood by care teams
* Good IT Skills including Microsoft and email applications
* Good communication skills to explain complex and sensitive information effectively in both written and oral to both patients, families, carers and other professionals
* Knowledge of pressure area care, nutrition and moving and handling with the ability to assess risk
* Commitment to improving and promoting palliative care services
* Motivated and committed to personal and professional development
Abilities
* To respond to difficult situations in a sensitive manner and have the ability to deal with difficult emotional situations
* To be an effective advocate for the patient within multi-disciplinary teams across all healthcare settings
* To work shift patterns on a rota basis including nights, weekend, bank holidays
* To be flexible in covering shifts at short notice.
* To effectively manage own time and workloads
Experience
* Experience of providing personal care and support for patients within care in a hospital, hospice or community setting
* Experience of working in a person centred/personal outcomes approach
* Awareness of palliative care issues
* Recent Palliative Care Experience
* Experience of dealing with vulnerable adults in a care setting
* Experience of working collaboratively within multidisciplinary teams and the ability to form good working relationships at all levels including volunteers
