The role involves working with people in a variety of settings such as their own homes, supported living, day opportunities, and the wider community. You will work with multi-disciplinary health clinicians under their supervision to provide interventions for specific tasks delegated by health practitioners.
Key responsibilities include:
1. Working as part of a Learning Disability multi-disciplinary, integrated team.
2. Supporting service users with a range of priority health needs under the supervision of a health practitioner.
3. Participating in providing health duty with the support of health clinicians.
4. Participating in regular planning and review meetings with the health practitioner supervising pieces of work and maintaining quality and management of caseload within the supervision process.
5. Maintaining adequate records as required by existing procedures and entering appropriate details on the service user's case notes as necessary.
6. Attending and participating in wider team/care plan review meetings as appropriate.
7. Working with service users to facilitate access to primary and secondary healthcare services.
8. Helping to identify early signs of mental and physical health decline by monitoring the service user's progress, level of functioning, and mental state, and alerting the health practitioner involved in the individual's care.
9. Being able to access the local area as this is a community-based post which will involve a high amount of travelling.
