Job summary Non-Medical Prescriber Dementia Intensive Support Team, West Suffolk We have an opportunity for a Non-Medical Prescriber to join the Dementia Intensive Support Team in West Suffolk (DIST). This is an exciting time to work in older People's community services and we are looking for someone who is passionate about working with older people to help DIST evolve and grow as part of wider service transformation. DIST works in a collaborative and supportive way which puts our service users at the heart of everything we do. We are looking for an NMP colleague who will bring their skills and expertise to the team to this MDT approach. Our dedicated team work intensively, for short periods with people with Dementia in the community, visiting their homes, often in crisis, working to stabilise and enable people to continue living independently and within their own homes. We care coordinate but support each other as a team, with weekly clinical team meetings and work collaboratively utilising all of the different skills among the team. Managerial and clinical supervision are embedded in the team. We are an inclusive service and welcome applications from people of all backgrounds. Main duties of the job Formulating treatment plans to provide a holistic service to meet mental health and physical health needs. To complete comprehensive assessment including assessment and management of risk. Undertaking urgent medication reviews for those in a crisis. Exploring alternatives to admission and advocating for intensive community-based/home treatment options. To provide advice, support and guidance where required to the MDT. This role is largely community visits within the urban/rural areas West Suffolk. To support staff development and act as a role model to the team, such as demonstrating the behaviours required in line with our Trust values. Take part in Quality and Safety Reviews within the Trust. Undertake Line Management Supervision within the team for an agreed number of staff.
