Job summary The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the PCN leads and management team in the reviewing of clinical policy, practice and procedure with respect to the PCN enhanced access and hub services. Apart from these clinical responsibilities, you will also work as an autonomous practitioner, responsible for the delivery of practice nursing services for the PCN and where required within individual PCN member GP surgeries, working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. You will be a healthcare professional NMC/GPC/HCPC registered with a license to practice, be an independent prescriber. Main duties of the job We are looking for an Advanced Clinical Practitioner Nurse who must; have a masters degree level in the relevant area of expertise; be working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice have current NMC registration.
