Job summary The Respiratory Clinical Nurse Specialist/Practitioner is a key member of the senior clinical nursing team, dedicated to delivering personalised, high-quality care to individuals and their families affected by respiratory conditions. This role requires advanced clinical expertise, strong communication skills, and a compassionate approach to identifying and addressing the needs of patients with chronic respiratory diseases and those requiring home oxygen therapy. Working both independently and as part of a multidisciplinary team, the specialist/practitioner plays a pivotal role in leading care and service delivery for patients with COPD and home oxygen needs, ensuring a holistic, patient-centered approach in both hospital and community settings. This includes recognising and respecting social, cultural, and spiritual differences while promoting the best possible outcomes for patients. Main duties of the job As part of the Respiratory team the CNS/Practitioner will be responsible for the co-ordination and care of patients with respiratory illness to ensure they receive the highest quality care. You will be a senior member of the specialist workforce and use advanced assessment skills and knowledge to review patients, supporting the Virtual Ward model to ensure the patients receive the same or improved clinical outcomes and patient experience, compared with an equivalent inpatient stay. You will ensure that patients discharged from hospital following an admission with their chronic lung condition will have appropriate care and support to avoid readmission, advise and assess patients in the community for home oxygen therapy and respond promptly to community requests for admission prevention for patients exacerbating with their chronic lung condition.
