We are looking to recruit a full time Matron to support our integrated Virtual Ward and Urgent Community Response team now known as our ‘Acute care at Home service’ providing care to patients in their own home. The postholder will provide professional, clinical and operational leadership, acting as a role model, who inspires motivates and empowers clinicians to provide the safe, effective delivery of service requirements within the wider operational team. You will work collaboratively with the senior clinical and operational team to support the integration and collaborative working between the Urgent Community Response team and the NHSHome team as well as working alongside our existing team of Matrons who provide operational leadership to our community nursing and specialist community nursing services. You will work alongside the multiprofessional team which includes our community Integrated Therapy service and external stakeholders responsible for delivering safe, responsive and effective care to the population of Swindon between the hours of Monday to Friday. This will facilitate continuous review/peer review, benchmarking, improving nursing services and ensure clinical effectiveness. Contribution to business planning is pivotal to ensure services are leading edge and delivered by a skilled workforce. Provide clear Operational leadership and take responsibility and accountability for specific nursing and Allied Health Professionals services, during both regular and unsocial hours. Provide assurance to the Deputy Divisional Director of Nursing that there is continuous improvement in the delivery of clinical services, in relation to national standards and directives and the local Strategies, ensuring that safety and quality of care are the underpinning principles. Be an integral part of the senior nursing group working in conjunction with senior management team and influencing the corporate nursing agenda and future direction. Support and drive the implementation and maintenance of safety, quality, clinical effectiveness and patient experience through the implementation and monitoring of key standards and outcome measures to deliver safe and effective harm free care. As part of the service line triumvirate, act as the expert in delivering the quality and safety agenda that is unique to nursing, implicit in this is continuous improvement and a reduction in harm. Work with clinical leaders to achieve a culture of harm free care by taking a visible leadership role Utilise expert analytical and critical thinking skills to ensure that care and service improvement agendas are innovative, contemporaneous and clinically effective. Develop and utilise monitoring tools and techniques to provide assurance against key performance indicators and care standards both within the organisation. Analyse data and trends to establish early recognition of areas of concern and provide action plans to rectify issues. Develop a culture of listening, whereby patients, relatives and carers are central to service delivery and development. Coordinate and facilitate the development, implementation and review of standards, policies, procedures, competencies and clinical guidelines with the multi-professional team. Investigate serious incidents and complaints to identify trends and implement action plans as a result. Ensure lessons are learnt and actions are taken to improve care standards and achieve zero harm. Ensure the governance agenda within the service line encompasses continual quality monitoring and improvement that focuses on reducing risk and developing efficiency and quality in care. Utilise relevant dashboard data to monitor and develop clinical services. Ensure the appropriate use of the Infection Prevention and Control Team, to ensure practice adheres to national standards and emerging advances in care. Undertake inspections of clinical areas across the Trust to monitor and maintain standards. Influence and design complex care and discharge processes using expert leadership, analytical and communication skills. This must deliver consistently safe and timely discharge planning. Ensure that services are continually developed and reviewed, to deliver pathways of care that are effective and efficient, delivering Trust objectives and strategy. The Ideal Candidate Current NMC registration – level 1 RGN Educated to degree level Masters level qualification or equivalent study at level 4 Management qualification or training certificate In depth experience of leading and managing teams of nursing staff across different levels within the Community/ Primary Care Setting Significant experience of working in a multi-disciplinary environment with a range of health professionals across Primary Care. Evidence of leadership and management experience. Experience and understanding of Caseload Management. Experience and understanding of a range of HR issues, including performance management, disciplinary, appraisals and clinical supervision. Demonstrable skills in leading and managing change with teams. Awareness of CQC regulations for Community Services. Sound knowledge of Safeguarding processes, DOLS, the Mental Health Act, Mental Capacity Act and their implementation Advanced communication skills Advanced negotiating skills A proven ability to deal effectively with patients, their families and carers and the general public Skills in working with a range of professionals across differing organisations and boundaries Ability to work in a pressured environment and balance competing priorities of services and staff Ability to deliver sustained quality improvement About The Company We change lives by transforming health and care.
