Please be aware this is a Long Term Temporary post to start ASAP.
To be an effective member of the SEND Team (with a designated caseload) supporting the effective management of an efficient statutory assessment service for children and young people. To liaise with professionals and other support services to provide appropriate and sufficient information and advice to ensure the statutory assessment process and timelines are met when new Needs Assessments are submitted. To use advice submitted by all professionals to create an EHCP for individual children and young people, undertake co-production meetings with families and education settings to support finalising plans.
Responsibilities:
1. To monitor and track new Needs Assessments as they are submitted, liaising effectively with all services to ensure all advice and information is completed in a timely manner.
2. To use information submitted from all parties to write an effective Education, Health and Care plan for children and young people within the statutory time frames.
3. Maintain effective record systems to enable the preparation of necessary reports to contribute to the Education, Health and Care plan.
4. Lead on co-production meetings to communicate with all agencies, parents/carers, and children within the statutory timeframes.
5. Be aware of and comply with the statutory requirements SEND Code of Practice, departmental policies, equal opportunities, and other regulations and procedures in order to ensure statutory and Departmental requirements are met.
6. Ensure that good communication and liaison is sustained throughout the team and with relevant external people, so that appropriate resources may be identified and mobilised for the benefit of the individual.
7. Link effectively with locality leads at all stages to ensure clear communication throughout the process.
