JOB DESCRIPTION AKG Group UK Company: Healthfind Position Title: Health Coach Job Level: 7 Role Status: Full Time / Part Time Reports To: Head of Operations Roles Reporting to this Position: None Primary Objective: The role of the Health Coach is to provide services with the aim of empowering your clients to achieve their goals and maximise their independence, helping people with long-term health conditions improve their health management and return to work. Key Relationships/Interactions Internal Your team Collaborating with other health coaches in your team is essential for fostering a supportive and cohesive environment that maximises the impact of your collective efforts and ensures continuous improvement. Employment Coaches: Collaborate with employment coaches and Employer Solutions Consultants who help individuals with job search strategies. By combining your health coaching expertise with their career guidance, you will offer a comprehensive approach to enhancing employability. External Participant/Individuals: Your primary focus is on your participants, the individuals seeking to improve their employability through better health and wellness. You'll work closely with them to understand their goals, challenges, and specific health barriers preventing them from going in to employment or sustaining it. Building a strong rapport with each participant is crucial for effective coaching and support. Professional Networking Events: Attend networking events and job fairs, to promote Healthfind and our health coaching services and make connections with potential clients and employers. Medical Professionals and Therapists: In some cases, your participants may have specific medical conditions or require specialised care. Building relationships with medical professionals such as GP’s, local community-based services and therapists can ensure a holistic approach to supporting our participants health needs. Key Responsibilities The post holder will provide skilled Triage assessments using Outcome Star and deliver evidence-based health and employment focussed interventions to participants on the programme, utilising their specialist skills, knowledge and experience to provide participants with advice, guidance and support tailored to the participants’ individual needs. The post holder will take a holistic approach to long-term health condition management and work closely with Employment Coaches, other Jobs 22 employees and health services to help people improve their lives. This role is responsible for providing individuals with work-focused health services (including access to high quality, evidence-based and cost-effective health solutions) to support performance and improve the opportunity for individuals to reach their potential. Direct Delivery to Participants Conducting triage assessments with participants. Providing face to face and telephone or video based interventions to participants. Delivering workshops with groups of participants. Monitoring participants’ health and wellbeing in individual and group work, reporting any significant changes which may impact positively or negatively on a participant’s recovery and return to work to the employment specialists. Producing tailored health action plans for each participant referred, focusing on improving their health and moving them closer to work. Offering advice and guidance to participants with regard to their health conditions and empowering individuals to manage their health more independently and effectively. Providing return to work guidance and offer in-work support as required, Delivering lifestyle, health specific, and psycho-educational workshops. Ensuring participants are able to access appropriate self-help material. Referring participants to specialist services, agencies and organisations where appropriate. Joint working with other operational roles and other health professionals. Assisting and supporting as many participants into full time sustainable work as possible Maintaining Accurate Records Ensuring that all participant related activities are recorded accurately and appropriately on our systems in a timely manner, as per KPIs and professional standards. Updating and uploading participants’ health action plans onto the system as appropriate after appointment and group sessions Recording all other supporting documentation as appropriate onto participants’ files Providing statistics in relation to performance by the end of each month, and for quarterly reviews, as per KPIs. Manage Risk Related Issues Appropriately Escalate any risk issues as per Healthfind Safeguarding Policy. Ensure your Senior Health Coach or Head of Operations is aware of any risk issues which have been escalated with regard to Safeguarding. Stakeholder Management Demonstrating excellent communication with the health and wellbeing team and all other operational staff. Promoting and generating referrals to the health and wellbeing service to engage and maximise uptake. Working with Head Operations and the wider health team to build relationships with external organisations such as the NHS and specialist providers of health and wellbeing related services. P roviding, upskilling or training to operational staff to continually improve the service offer to participants and operational performance through ongoing support and through induction. Performance Targets Regularly review outcomes and functional improvements to ensure efficacy of intervention. Implement strategies to ensure the continued engagement of clients. Responsibility for maintaining your caseload through referrals from sites. Measurement against interventions carried out each day Continuous Professional Development (CPD) Participating in 6 weekly 1-2-1 review meetings to discuss your performance in the role and delivery of the health and wellbeing service. Engaging in monthly supervision with as per the clinical supervision guidelines.(this is provided through external body) Participating in an annual professional development review to identify areas for ongoing personal and professional development. Participating in training and development opportunities provided. Taking ownership for your CPD and maintaining your professional registration. Essential/Desirable Skills, Knowledge and Experience You will work with adult clients from all walks of life, with a broad range of abilities and often the very vulnerable with complex needs. The ability to work via digital video, over the telephone and face to face is essential to the role. A registration with the NMC or BACP or equivalent would be desirable but not essential. Degree/equivalent in mental health nursing or adult nursing, or qualification in counselling or occupational health or similar. NMC or BACP or equivalent registration. Passion for helping a diverse range of clients. At least 2 years’ experience would be desirable. Experience working within Disability or Employability Sectors, specifically using an assessment and intervention service model is desirable. Strong communication and presentation skills, coupled with an ability to maintain productive relationships Enhanced criminal record bureau check (Disclosure & Barring Check).
