We have an exciting opportunity to join a dynamic and forward-thinking Rheumatology team. We want to recruit an energetic and innovative Rheumatology specialist nurse into the team. This role will involve autonomous, independent clinical practice. With a background of nursing, the post holder will work closely with Consultants Rheumatologist, working towards improving service for patients.
Whilst this post is advertised as AfC band 7 based on meeting essential criteria of holding PACR (or equivalent physical assessment qualification), PADRAP (or equivalent physiology for advanced practice qualification) and independent prescribing qualification, candidates not fulfilling these elements may be considered on a developmental basis. In which case, candidates would be paid at AfC band 6, pending successful completion of the required training modules.
· Work closely with Consultant Rheumatologists and clinical nursing staff within the Neuromusculoskeletal directorate (NMSK) and other directorates within the trust towards improving service for patients.
· Independent nurse led follow up clinics for patients with inflammatory, autoimmune diseases for medication review/changes and pain management.
· To contribute to the education of patients with inflammatory, autoimmune, and degenerative diseases, their carers, GP’s and other outside key roles.
· Participate in an managing the patient advice line .
· Responsible for (coordinating and/or delivering) Rheumatology pathway management.
· Responsible for (coordinating and/or delivering) Rheumatology training at the North Bristol NHS Trust
· Increase patient satisfaction and provide a competent and seamless service.
· Be responsible for enhancing the continuity of care as planned by the Rheumatology consultants.
· Under supervision/facilitation of the Consultant team/nursing team, to develop ways of working to enhance the care and communication within the service. Utilising advanced practice skills where appropriate to drive the service forward.
· Development of robust protocols/procedures for practice within the service to satisfy clinical governance expectations. · Attend the Rheumatology MDT
