We have a great opportunity for an enthusiastic, motivated clinician with excellent communication and clinical skills to join our Memory Assessment Service in High Weald, Lewes and Havens. You must be enthusiastic, self-motivated, flexible in approach and able to work as part of a multidisciplinary team. You will possess excellent communication skills, a sound clinical knowledge base and a commitment to caring for patients.
As part of the team, you will be required to carry out cognitive assessments within a clinic or patients' home, refer/signpost to additional support/health services, produce accurate assessment records and provide relevant advice and guidance to our patients/carers/family members.
This post will suit a Registered Mental Health or a Registered Occupational Therapist.
The post holder will work across three sites: Grove House Crowborough, Millwood Uckfield, and Hill Rise Newhaven.
The post is 37.5 hours per week, Monday to Friday 9.00 am to 5.00 pm (excluding Bank Holidays). Part-time hours will be considered.
You will need to be able to travel across sites.
Key Responsibilities:
* Work as part of a multi-disciplinary team to screen and triage referrals received within the service.
* Complete a specialist assessment of clinical presentation and symptoms.
* Provide assessment information for the weekly multi-disciplinary team meetings to support the diagnostic process.
* Complete referrals to organisations such as Adult Social Care and DVLA.
* Produce accurate records on our patient information system.
