We are offering a fantastic opportunity to work within a partnership team to help increase people’s health, wellbeing and active participation within their local community.
This role is funded through the Pier Health PCN Social Prescribing Service (a partnership between Pier Health, CANS and Alliance Homes).
You will take referrals from the Pier Health Primary Care Network’s footprint, including GP surgeries and health care professionals. Develop trusting relationships and co-producing personalised support plans. Help customers to live more independently and ensure they can access resources within their local communities, whilst using a “what matters to me” strengths-based approach.
For this role you must have the ability to build trust and rapport with people and hold good influencing and negotiating skills. You also need to be able to work with initiative, manage your own workload and be able to provide monitoring information to assess the impact of the service.
Due to the nature of this role, you will need to be able to drive with access to your own vehicle. The successful candidate will also be subject to a satisfactory Enhanced DBS check.
