What you will do:
1. Conduct comprehensive Care Act assessments to ensure individuals' needs are met and independence promoted.
2. Engage with hospital discharge pathways, contributing valuable insights at MDT meetings both on the wards and within the community.
3. Collaborate effectively with ward staff, healthcare providers, and social care professionals to coordinate seamless, person-centred discharges.
4. Support the Discharge to Assess (D2A) model by completing assessments in community settings or bed-based placements.
5. Partner with the Reablement Team to facilitate timely access to necessary services post-discharge.
6. Address safeguarding concerns as needed, conducting enquiries under Section 42 of the Care Act.
7. Maintain meticulous case records and documentation in adherence to local authority standards.
8. Participate in team duty functions, sharing knowledge and contributing to the team's collective expertise.
9. Champion a strengths-based approach, ensuring alignment with person-centred values and current legislation.
