Our Crisis Resolution and Home Treatment teams are multi-disciplinary and provide a community-based mental health service. CRHTT aim to provide a safe and effective home-based assessment and treatment service as an alternative to in-patient admission. We can facilitate early discharge from the ward which provides an opportunity for patients to receive care and support in their own homes, which is the least restrictive environment.
The workload is team-based, and our team members do not carry a specific caseload themselves; instead, they share one as a collective unit.
The teams are resourced to provide care and brief crisis intervention 7 days a week for people experiencing a mental health crisis. The Service supports adults from the age of 18, which is recovery-focused and works in collaboration with clients while supporting carers dynamically according to individual needs.
The response will include assessments at MHA's and home visits. Staff will undertake rapid assessments and then either refer or signpost to other services. Out of hours activity will be coordinated through the Mental Health support Line and responded to accordingly. Merton Crisis Resolution and Home Treatment Team offers an alternative to acute psychiatric admission, and the team acts as 'Gatekeepers' to the acute ward, working closely with the Trust's inpatient wards to help action this effectively.
Main duties of the job
We are currently looking for full-time Nurses Practitioners Band 6, who are motivated and committed to being part of high-quality services working within a multi-disciplinary team, including recovery support and peer support workers. We are looking for practitioners who will be committed to delivering high standards of patient care that is evidence-based.
The successful candidates will be based at either Springfield Hospital in Tooting or The Wilson Hospital in Mitcham and will be expected to work a combination of early and late shifts, which will include weekends. In addition to the benefits of working as part of a multi-disciplinary team, the right candidates will be working mostly independently. You must possess good communication skills, be organized, and have good time management. A car driver/owner is essential. We are committed to continuous professional development, and you will receive regular supervisions to adapt to the needs of the service and to work towards PADR indicators.

Job responsibilities
The Deputy Nursing Team Leader/Clinical Charge Nurse/Community Psychiatric Nurse/Practice Development Nurse role is suitable for Registered Nurses who have successfully completed preceptorship and have registered as an approved NMC mentor.
The post holder will deliver, or advise on the delivery of, high-quality safe nursing care, ensuring consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.
As a Deputy Nursing Team Leader/Clinical Charge Nurse/Community Psychiatric Nurse/Practice Development Nurse, you will promote and develop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care. You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment. Your professional and personal development will include further postgraduate study, and you will be encouraged and supported by your line manager to undertake Masters programmes.
Person Specification
Education and Qualification
* RMN (current NMC registration).
* Evidence of continuing professional and personal development.
* Full and current driver's licence and car owner (or willing to take part in car lease scheme run by the Trust).
* Mentorship, ENB 998 or equivalent.
* RGN.
* Diploma in cognitive behavioural therapy/psychodynamic counselling.
Experience and Knowledge
* Demonstrable experience of working with people who experience serious ongoing mental health problems in a community or outpatient setting post qualification.
* Experience of working in a multidisciplinary team.
* Experience of assessing and managing crisis with those with severe mental illness.
* Experience of working as a clinical supervisor.
* Experience of managing and supervising staff.
* Experience of contributing to policy and procedure development and implementation.

Salary: £37,338 to £44,962 a year per annum plus HCAs.
