Job overview
As part of the NHS Community Mental Health Transformation Programme, an exciting opportunity has arisen for a Band 7 Mental Health Nurse Practitioner to work as part of the Community Urgent Care Response Teams in Primary Care, offering specialist Mental Health support to patients cared for within the community. The post holder will work closely with Physical Health Urgent Care Team colleagues in Primary Care, providing advice and support for patients with complex or challenging Mental Health needs, and where appropriate, provide initial assessment and first line treatment followed by signposting including direct access to support from Older Adults Enhanced Community Mental Health Teams. The post holder will provide a high standard of assessment including risk management and treatment for patients referred to the service, including patients with Complex Mental Health needs.
Main duties of the job
* The post holder will be required to exercise a high degree of professional autonomy and critical judgement, making professionally autonomous decisions by utilising knowledge, skill, and attitudes gained through studying and experience and by maintaining competencies via medical/nursing supervision.
* The post holder will provide specialist Mental Health Skills advice and guidance to health professionals and colleagues, including care homes and those patients within Physical Health Virtual Wards, offering expert clinical support including initial assessments and first line treatment.
* The post holder will provide education and training for professionals in understanding the needs and support for Older Adult Mental Health patients, including families and carers, to improve health outcomes and offer care closer to home.
* The post holder will be required to undertake Independent Non-Medical prescribing where clinically appropriate, utilizing NICE Guidance & Trust prescribing policy, or be willing to undertake NMP training course once in post.

* To undertake Independent Non-Medical prescribing where clinically appropriate utilizing NICE Guidance & Trust prescribing policy.
* The post holder will work closely with Physical Health Urgent Care Team colleagues in Primary Care providing advice and support for patients with complex or challenging Mental Health needs, and where appropriate provide initial assessment and first line treatment followed by signposting including direct access to support from Older Adults Enhanced Community Mental Health Teams.
* To provide support with care planning for individual patients providing education and practice development with referrers.
* To provide education and training for professionals in understanding the needs and support for Older Adult Mental Health patients, including families and carers, to improve health outcomes and offer care closer to home.
* To provide strong and effective clinical leadership, providing specialist mental health skills, advice and guidance to nurses in both the statutory and private sectors, and initiate change as a result of audit and evidence-based research.
* To exercise a high degree of professional autonomy and critical judgement, making professionally autonomous decisions by utilising knowledge, skill, and attitudes gained through studying and experience and by maintaining competencies via medical/nursing supervision.
* To undertake appropriate risk and other assessments, ensuring they are completed and documented in a timely manner in accordance with Trust policies.
* To develop coordinated links throughout the Trust, and a wider health and social care community, including social care and voluntary/3rd sector services in meeting the needs of patients and families during the referral, assessment, and signposting phase for secondary Mental Health referrals.

* RMN
* Post Graduate experience working in Older Adult MH Community Services
* Specific Training / Experience in working with Older Adult Mental Health
* Effective Risk Assessment and Management skills
* Advanced Knowledge and application of current best practice in OAMH assessment
* Ability to build & maintain effective working relationships with team and partner agencies and other health colleagues
* Ability to work as part of a team or independently
